I had a MRT Food Allergy Panel ran. The results said, that I needed to avoid Cow's Milk. Does this include butter? Whey Protein?

Even more good news for the lactose intolerant; organic butter - no problem!
It is typically not the fat portion of the dairy that causes the reaction; it usually the protein portion of the dairy.
A high quality, organic whey protein isolate should not cause any reaction either.
I recommend Poliquin's Whey Stronger. Keep in mind if your MRT came back with Vanilla or Cocoa, you have to go Natural flavor. I have recently had several clients that have sensitivities to Vanilla or Cocoa and 1 unfortunate that can't have either one.

So - if your food allergy test comes back showing a cow's milk and yogurt sensitivity then is it always lactose intolerance?

On an allergy test they are measuring anti-bodies, which means there is a good chance the proteins in dairy could be getting through the GI tract rendering you with leaky gut syndrome. The micro-villi (brush border) have been blunted, so you are missing the lactase enzyme to break down lactose (milk sugar).
Going dairy free while you rebuild your GI tract on a gut protcol would be your first step to healing.
Check out the question regarding anti-biotics and pro-biotics. Pro-biotics and digestive enzymes are also very helpful to leaky gut (which is part of a gut protocol).
